We provide service users with care and support in a nurturing, safe environment, where they can thrive during their recovery journey to wellness living in the community.
We provide this through 4 different Pathways of Care:
Service users living in these homes have their own Tenancy and are enabled to maintain or develop independent living skills and typically stay for a couple of years. They are referred following a Needs Assessment which determines the care and support they receive, described in a Personalised Support Plan. During their stay, these locations are considered the Service Users’ home and staff’s approach reflects this.
These homes provide short-term placements for service users discharged from hospital to receive support to maintain or develop daily living skills whilst they integrate back into the community. As one service user put it, these homes provide a “beautiful soft landing between hospital and home”. Length of stay is typically weeks to a few months.
These homes provide support 24-hours a day for users experiencing a mental health crisis who are already living in the community;. Service users stay in a Crisis House for typically 1-2 weeks and will then return to their previous accommodation. Staff’s focus is on providing a safe space for service users to “recuperate, reflect, rest and heal” as one individual put it, and continue to the next stage in their Recovery Journey.
These are 24-hour housing-related support services with accommodation for homeless residents and Young People. Support is focused on encouraging opportunities for service users to participate in activities and develop new skills, building upon their strengths, interests and aspirations to enable them to move on to accommodation with their own tenancy. Typically, this includes into the Private Rented Sector, Local Authority Housing, or other supported Housing Schemes.
“I enjoyed my stay at Hassocks. Engagement and support from staff and as well as engagement with other clients gave me the distraction tool and escape option I needed. I would definitely recommend others to grab the opportunity if presented to them to agree to being referred to Hassocks”
“I feel anxious but if I can have good support like I am having here I will keep trying”.
“Hassocks gave me the space I needed from home, and I enjoyed talking and mixing with others and not feeling low as I had people around me all the time. I feel better going home now …”
“I thank you from the bottom of my heart for everything you have done for me, after I get to work, I want to invite you for a coffee to be honest”
Thank you very much for all your care and support! I am indebted to the team who took extraordinary efforts to steer me the right direction and ensure living independently. Many thanks to Mrs OA (my key support worker) for her professional, intuitive and or dedicated care to achieve the outcome today!
“To whom it may concern, I'd like to express my gratitude and appreciation to the staff at Kempshott Road [step-down home with 3 CoS staff]. My son moved in on 1 June of this year, and since then he has been stable in his wellbeing, and this has mainly been due to the staff. The staff at Kempshott are very kind, patient and understanding. They do their absolute best to maintain a healthy, clean, and calm environment. They treat visitors and tenants with the utmost respect, and when I have had instances to be concerned about my son (when I've not been able to reach him on his phone), the staff have been quick to reassure me all was well. When my son had a mini crisis, they able to gently assess his needs without causing him additional concern. Staff are very kind and always very reassuring. I cannot fault this service and I'm very happy to know my son is safe, day and night. This service is much needed, but what makes it work so well are the staff!! Thank you”
